Job Description Summary

POSITION SUMMARY:

The Specialized Case Manager is responsible for providing intensive case management services to OC CalWORKS Participants. They will also be responsible for providing Cal-Learn, Domestic Abuse, and Family Stabilization Case Management Services, and Mutual Client Case Management Services with Child and Family Services.

QUALIFICATIONS:

Education: Bachelor’s degree from an accredited college or university, preferably in a human services related field, or four (4) years of experience in employment services, or human services.

Skills/Experience required:

  • Minimum of one (1) year work experience required in social casework in a public or private organization.
  • Bilingual capabilities in Spanish, Vietnamese, or Arabic preferred.
  • Ability to interact successfully with program participants, project staff, clients and subcontractors
  • Computer proficiency in Microsoft Office programs including Excel
  • Use database management programs
  • Establish relationships with employers and community groups
  • Demonstrate ability to work with culturally diverse populations
  • Excellent organizational, interpersonal, written, and verbal communication skills
  • Perform comfortably in a fast-paced, deadline-oriented work environment
  • Ability to successfully execute many complex tasks simultaneously, working as a team member, as well as independently
  • Ability to develop, evaluate, and implement a case management plan meeting all milestones
  • Ability to work with customers in a caring and respectful manner and with due understanding of and consideration for their unique circumstances
  • Excellent organizational, written, and verbal communication skills
  • Ability to perform comfortably in a fast-paced, deadline-oriented work environment
  • Ability to work as a team member, as well as independently
  • Must be able to remain in a stationary position for an extended period of time
  • Occasionally lifts, carries, or otherwise moves items weighing up to 25 pounds
  • Work is constantly performed in an office environment

    JOB RESPONSIBILITIES:

  • Provide intensive case management to a caseload of specialized CalWORKs Participants working towards the goal of self-sufficiency and unsubsidized employment under the general supervision of the Case Manager Supervisor.
  • Provide Cal-Learn, Domestic Abuse, and Family Stabilization Case Management Services, and Mutual Client Case Management Services with Child and Family Services.
  • Provide Participants with a clear understanding of the CalWORKs/WTW program and services.
  • Provide encouragement and support to Participants to ensure successful participation in each step/component of the CalWORKs/WTW program.
  • Serve as primary linkage between Participant and program services.
  • Provide continuous motivation to keep the Participant moving toward employment goals.
  • Interview Participants to elicit basic program information for entry in computer information system(s).
  • Review Participant information for exemption from the CalWORKs/WTW program and good cause for non-participation.
  • Advise the Participant of program choices and refer them to the appropriate CalWORKs/WTW Activity based on Participant evaluation, and Orange County CalWORKs/WTW Policies and Procedures.
  • Assess for Supportive Services needs, make appropriate referrals for services and coordinate ongoing services needs on a regular basis with County staff per COUNTY Policy.
  • Monitor progress of the Participant through the CalWORKs/WTW Program.
  • Conduct good cause determination.
  • Maintain accurate case records for the Participant in the computer information system(s).
  • Re-engage and educate non-compliant Participants on the benefits of participating in the CalWORKs/WTW Program.
  • Re-evaluate Participants for previously unreported and/or undetected barriers to participation and assess their current knowledge, aptitude, still, and experience for work.
  • Provide Participants intensive outreach services including face-to-face, off-site, and home visits.
  • Conduct follow-up Participant meetings and appointments to validate program activity and continued progress.
  • Complete monthly contacts as required.
  • Complete case narrations as required.
  • Complete and submit reports as required.
  • Perform other duties as may be assigned by management.

    Essential Duties and Responsibilities:
    - Provide case management services to a caseload of assigned program participants experiencing severe barriers to employment.
    - Develop and use expertise to address special issues affecting participants who are on long-term Cash Assistance; have a history of sanction or program non-compliance; or ex-offender status.
    - Consider barriers to employment in making program placement decisions.
    - Provide job readiness screening and assessment.
    - Assess participants need for Back to Work services and assist participants to develop Employment Plan.
    - Refer applicants and participants to appropriate program components.
    - Coordinate all project services provided to a participant.
    - Refer program participants to community resources, including long-term and emergency child care services; and provides follow up as to the effective provision of service.
    - Provide employment counseling and job leads to participants to ensure job placement and retention.
    - Provide support to ensure stability in employment and coordinates ongoing retention activities for employed participants if necessary.
    - Initiate non-compliance procedures and follows up on process for participants who fail to meet program requirements.
    - Participate regularly in work groups and committees aimed at evaluating customer needs, improving services, and implementing new service strategies.
    - Perform other duties as may be assigned by management.

    Minimum Requirements:
    - Comprehensive knowledge of and established relationships with the local employment market and labor market needs.
    - Ability to develop, evaluate, and implement a case management plan meeting all milestones.
    - Ability to work with customers in a caring and respectful manner and with due understanding of and consideration for their unique circumstances.
    - Excellent organizational, written, and verbal communication skills.
    - Ability to perform comfortably in a fast-paced, deadline-oriented work environment.
    - Ability to work as a team member, as well as independently.
    - Bachelor’s degree from an accredited college or university in related field, equivalent experience considered in lieu of degree (four years applicable experience may substitute for degree) and 1 year related work experience required.
    - Experience in marketing employers and working with business professional community.
